A Utah woman died Wednesday morning after she became caught by an industrial mixer and was pulled into the device at a grocery store bakery, according to the Salt Lake Tribune.
Dean Carriger, a sergeant with the Sandy, UT Police Department—a suburb of Salt Lake City—told the Associated Press the woman was loading ingredients into the large bowl mixer when it is believed a piece of her clothing became tangled with the equipment as she reached over the top. Her death is believed to be an accident.
OSHA will investigate the incident to determine if any safety measures were violated.
